Qualifications and Experience:

• Licenses and Certifications: Copies of relevant medical licenses for the practitioners offering FXN services (e.g., Medical Doctors, Physical Therapists, etc.)

  • Must be Board Certified by the American College of Functional Neurology or the American Chiropractic Neurology Board. 

  • Must have a permanent business location with a physical address.

  • Must provide proof of hourly clinical charge rates.
